Privacy notice for this appointment request form
This notice explains what the form on atlanta.havcog.com collects, why, where it goes, and the choices you have. It supplements the practice's Notice of Privacy Practices.
Who we are
This page is operated by Heart and Vascular Care of Georgia ("the practice"), an independent, physician-owned cardiovascular practice with offices in Atlanta, Newnan, and LaGrange, Georgia.
What this form collects
Your full name, phone number, email address, the best time to call you, your insurance status (insured, self-pay, or not sure), and your consent choice. The form doesn't ask about symptoms, conditions, or any other health information, and it has no message or comments box, so there is nowhere to type health details; our scheduling team asks what you'd like to be seen for when they call. We also record the date and time you sent the form and, if you arrived from an advertisement, the campaign identifiers attached to that link (for example, utm_source and utm_campaign) so we can tell which ad brought you here. We don't record advertising click identifiers or your IP address with your request. We do store a one-way hash of your browser's identification string (its user agent), used only to detect automated abuse; it can't be turned back into anything that identifies you.
Why we collect it
To contact you about scheduling an appointment, and to understand which advertising sends patients to us. We use your information for these purposes only.
Where it's stored
Your request is sent over an encrypted connection and stored in the practice's own account with Amazon Web Services (AWS) in the United States. It's encrypted at rest with a key controlled by the practice. AWS has signed a Business Associate Agreement with the practice, as HIPAA requires.
Who sees it
Your request is emailed over an encrypted connection to the practice's scheduling staff and is available only to workforce members who need it to contact you. We don't sell your information, and we don't share it with advertisers, data brokers, or other third parties for their own purposes.
No tracking on this page
This page contains no third-party analytics, advertising pixels, embedded maps, externally hosted fonts, or scripts. No outside company receives information about your visit or your request from this page. The advertising platform that showed you the ad may collect information about your click on its own site before you arrive here; that collection is governed by that platform's privacy policy, not ours.
How long we keep it
Requests are kept only as long as needed to schedule your visit and as required by the practice's records-retention policy. If you become a patient, relevant information is added to your medical record and kept under that policy.
Your choices
You can ask us to delete your request or stop contacting you at any time by calling (770) 650-3434. You can opt out of text messages by replying STOP.
Your HIPAA rights
If you become a patient, your health information is protected under HIPAA and the practice's Notice of Privacy Practices. See also the HIPAA notice and the website privacy policy.
